At 4:00 PM (EST) on 9/22, the fix for this issue was deployed to all practices. Forms that had generated blank documents were regenerated with the correct data and should be viewable from the patient's chart.

We've identified the issue that is causing recent forms to not generate documents after being completed, or in some cases generating blank documents. All of the data is safe and still available, and we're currently finishing an automated fix that will go through and regenerate any of the completed forms that were effected.
